Even if a miracle happened and Brian was released from the hospital, he knew there was a high chance of his being sent back. He’d seen it happen. Patients who left Perkins had half the recidivism rate of prison inmates but were far more closely scrutinized. The slightest transgression, even a minor offense like petty theft, and they were back on the ward. Ex-inmates, on the other hand, could miss their parole meetings with little or no consequence, or even disappear into the community, escaping the justice system altogether.

Brian was sick of being medicated into a stupor, tired of making no progress toward release, and frustrated by the constant parade of patients who arrived at Perkins and got discharged while he stayed in the same place or went backward. Some of those patients had the advantage of wealthy and supportive families who could afford to hire private attorneys, or they were women, who seemed to move through the system more easily. As long as his doctors thought he was harboring secret reserves of hostility and paranoia, they wouldn’t even consider him a candidate for release. To make any progress at all, he had to comply—and compliance meant medication. There was a strong possibility, he reasoned, that he was going to turn into one of those old men wearing diapers who’d spent the last forty years slumped in front of the television.

This, he told himself, was not a life worth living.
